Before care can start, ALL admission paperwork needs to be completed in full, including signatures. Missing information and fees can result in a delay of start. All required paperwork can be found in the online application. The following fees are also due to begin care:

Registration fee: $25
First week of tuition BASED ON AGE GROUP /applies to first week of care
Two- week deposit fee BASED ON AGE GROUP/ non-refundable, can be used for last two weeks when a two-week notice is given.

Licensed by the MN Department of Human Services. A C2 license allows you to care for 12 children. Of those 12, 10 may be under school age. Of those children under school age, no more than 2 may be infants and toddlers, and no more than 1 may be an infant.

Provider shall not discriminate in relation to admissions based on race, creed, color, national origin, religion, or sex.

I carry Liability Insurance. If or when anything changes, I will inform you and a DHS form will be provided.

Childcare is open regularly Monday through Friday year-round from 6:30 am to 4:45 pm. IF YOUR CHILD IS ATTENDING FOR THE DAY, THEY MUST BE DROPPED OFF BY 9 AM. The 9 am drop off deadline does not apply to part time care.

Late drop-offs do not allow for late pickups.

The childcare program is open year-round, except for the holidays and vacations listed in my policy.

Part time care may be offered on a limited, case by case basis. Any differences in policies between full time and part time will be addressed in the specific related policy.

First time pickups for anyone who did not enroll the child(ren) will need to provide a photo ID listed under AUTHORIZED pick-up person(s). This does include a biological parent if we have not meet. Court papers will be needed if a parent is not allowed to be pick up their child, stating who has been granted legal and physical custody. I will only be able to release a child to others who have a car seat.

Reason for termination: I do not want to see a child leave my care, but there are cases in which I can give the care needed for your child and will terminate care; including but not limited to: behavior outbursts that cannot be managed, endangering other children or provider. Whenever possible, a two week notice will be given, however, it is not guaranteed and is based on the individual circumstances.

No child will be subject to corporal punishment or emotional abuse. “Corporal punishment” means the nonaccidental infliction of physical pain on a child by caregiver.

Corporal punishment includes, but to limited to, rough handling, shoving, hair pulling, ear pulling, shaking, slapping, kicking, biting, pinching, hitting and spanking. “Emotional abuse” means the infliction of verbal or phycological abuse on a child by a caregiver. Emotional abuse includes, but not limit to, name calling, ostracism, shaming, derogatory remarks about the child or child’s family, and threats which threatens, humiliates, or frighten the child. Food, light, warmth clothing or medical cannot be withheld from the child. Discipline and punishment shall not be delegated to another child. Separation of a child will be age appropriate to the age of the child and circumstance requiring the separation. The separation will be less than 10 minutes, and an area that is well lighted, free from hazards, ventilated, and open to the view of myself. A child will not be places in a locked room to separate. The child once calm will be reunited back into play. Toileting accidents are a teachable time, building the child up to success. Infants are shown modeling of appropriated behavior and re-directed when needed and shall not be separated for disciplinary reasons.

We have two cats and two dogs in our home. All are current on immunizations. Your child may encounter our pets during regular daily activities.

If your child suffers a minor injury, I will do basic first aid to the area. Washing it with soap and water etc. I will fill out and give you an accident report, so you know what happened. I will log it in my records and monitor the accidents eliminating or changing items or activities to ensure a safe environment.

If your child suffers a major accident, I will call 911, preform immediate First Aid/ CPR, and then contact you when it is possible. If I cannot reach you, I will then call your emergency contact person on your form. I will let emergency responders decide what actions will be taken. If your child needs to be taken to the hospital, an ambulance will be called, and this expense will be upon the parent. Any child that is seen by a physician after hours due to a possible accident during childcare hours must notify me immediately. I have 24 hours to report this to licensing.

A DHS written emergency plan, detailing our procedures in the event of fire, weather, or shelter in place order, will be provided upon starting my childcare. I will review this plan annually and you will receive one each year. The emergency plan has my location for evacuation and where to find us in the event of a fire. Fire Drill are done monthly, teaching children how to get out of my home and where to go outside. Tornado drills are done monthly, teaching children how to protect themselves and where in my home we will go. I have emergency supplies with flashlights, first aid kits and items needed for emergencies. I have posted important community numbers (police, fire, hospital etc.) and a contact list of all my family’s emergency numbers. When these change, please supply them so we can update emergency contacts.

I am a legal mandated reporter and will report any suspected child abuse and neglect. You will receive a copy of the mandated reporting policy for DHS in the orientation packed. Families have the right to report abuse while their child is in my care. My licensor contact information can be found on this paperwork.

I, employees, subcontractors, and any volunteers will never be under the influence of non-prescription medication or under the influence of a chemical that impairs the ability to provide service / care. Training on my drug and alcoholic policy will be done if/ when needed to the above people. If I suspect that a parent or anyone picking up your child is under the influence of drugs or alcohol, I will NOT release the child to that person.

I will do all that I can to protect your family’s privacy and will abide by the state privacy laws. I will keep all records and information about your child and your family private and confidential unless I have your written permission to reveal specific information. I ask that you respect the privacy of my family by not sharing any information about my family without my written permission.

My goal is to provide a well-rounded experience that will nurture enhance your child’s development, by offering a variety of activities and curriculum. My activities will explore a child’s physical, emotional, developmental, and social being. These are areas listed below.

Fine motor skills, large motor -outdoor games, music and movements, fitness fun, literacy and school readiness, story time, counting/math, exploring science fun, and emotion/conflict resolution.

We will celebrate holidays such as Halloween, Thanksgiving, Christmas, and birthdays. Your child’s participation is voluntary, based on your own belief system. Your child may bring a store-bought treat to help celebrate their birthday, but it is not required. I do have a TV in the playroom for appropriate programming during free time and music for singing and dancing fun.

Parents must submit in writing any changes of employment, address, phone numbers, physicians, and dentists, etc. This is extremely critical for the safety and well-being of your child.

If you need to bring your child into childcare later than your scheduled time or you will be picking up earlier than your scheduled time, please let us know. I ask that you schedule well baby/child doctor visits later in the afternoon so that the childcare’s routine is not disrupted. Please tell us as soon as you know there will be a change in your schedule so I can find the best way to accommodate that change.

I have 11 rules that I teach the children in our care and always enforce. Every day I reiterate these rules & talk about why these rules are important. I expect that you acknowledge, respect, and enforce our rules while you are present, as well.

1. We do not wear our shoes in the house.
2. We do not go out the door or gate without an adult.
3. We use inside voices indoors.
4. We use gentle touches. NO hitting, pushing, biting, grabbing, kicking, spitting, pinching.
5. We do not roughhouse, climb on, sit on, lift/carry, or pull on other children or pets.
6. We do not name call, tease, swear, or use potty language.
7. We use manners.
8. We have listening ears.
9. We have walking feet indoors.
10. We do not play guns or other forms of violent play.
11. We clean up. Everyone is expected to pick up.

When you show up at the end of the day, I expect that your child pick up whatever he/she was playing with before leaving out the door.

I expect that you DO NOT allow your child to open the gate or leave the back yard without you. ONLY open the gate when you are completely ready to walk out and have your child walk with you.

Smoking is not permitted on the premises. Breaking this rule will be grounds for termination. If you do smoke, DO NOT discard your cigarette butt on the premises. Smoking is not permitted in our home by anyone.

I expect items to get broken or damaged in the normal course of a childcare day. If the damage done by your child is excessive, deliberate, or the result of intentional misuse; you will be charged to replace the item(s).

Due to limited spots, fees are charged weekly regardless of attendance for any reason.

Payments can be made via cash, check, Venmo or through the online application, which is my preferred method. You can even set up auto payments within the app.

Payments are to be made in FULL one week in advance. If daycare is to be closed on your pay-date, all payments must be received the day before by 5:30pm. If your child is absent on your pay-date, you are still required to make your payment. There will be a $25.00 late fee added PER DAY until the payment is received in full. NO EXCEPTIONS. If payment is not received by the close of the business day on the Friday of the care week, your account will be delinquent.

Personal checks are accepted. However, a $35.00 fee for each non-sufficient funds check returned will be charged. Service will be REFUSED until payment is made sufficient, and the fee has been paid. Parents who have 2 non-sufficient funds checks will be required to pay through the online application.

I, the provider, will receive up to a maximum of 5 personal days per year for sick days, ongoing training, or other appointments, for which you WILL BE CHARGED. I will also take up to a maximum of 3 weeks of vacation per calendar year. I may take these days consequently or not. I will give at least a 2 week notice of planned vacation days.

I do not provide transportation to or from childcare. If your child has an appointment, attends pre-school, etc., you will need to arrange transportation.

The FULL responsibility for arranging back-up care is yours. Back up care is not provided by me.

It is my policy that all children wear closed toe shoes to play outside. Cheap little slip-ons would be perfect. Open toe shoes are a tripping hazard, an invitation for rock collections, and an invitation for injury from sticks on the ground. If your child arrives in unsafe or open toe shoes, your child will only be allowed to play on the cemented area with chalk or other appropriate outdoor toys.

During the warmer months, I try to get the children outside at most free play times in our schedule. Also, during the summer months, we play outside and play more games than we do curriculum. I keep mealtimes and nap times consistent. The client will be responsible for bringing diapers, extra clothes and bottles for their child’s use.

I DO NOT ALLOW food, candy, gum, etc. to be brought into the childcare. If you come into the childcare with any of these items, you will need to take them with you. If you choose to feed your child something on the way over, please make sure it is gone before you enter the childcare premises or leave it in your vehicle.

I do participate in the Food Program (“CCFP”). I serve nutritionally balanced, high-quality food from each of the basic food groups as defined by the United States Department of Agriculture at no additional charge. Your child will be offered the food at mealtime and is never forced to eat. I always try to incorporate at least one component at each meal and snack that I know each child will eat.

Meals and snacks are served only at the designated times. If your child is not here by the end of our serving time or picked up prior to it, it is your responsibility to feed him/her AT HOME.

Infants will be held during all bottle-feeding times. Bottles need to be labeled and will be washed after use. Introduction to solid foods will be a discussion with the parent(s) before starting. Please bring two labeled bottles for each infant. If you are feeding breast milk, please label it with your child’s name, and date it was pumped. I offer Similac Advanced and Gerber/table foods based on Food Program guidelines. If you prefer another specific brand, you are more than welcome to bring it in for your infant, at your cost.

Toddlers and preschoolers are provided with a designated cot, sleeping bag and pillow. Children are supervised during the nap/rest time and will be physically checked every 15 minutes while sleeping. When awake they will go to the quiet play environment for activities until all children are up. A child over a year of age may have a blanket, a security item for comfort. Blankets will be washed on a regular basis.

Non-nappers and short nappers are required to having a 30-minute rest and after this time they will go to the quiet environment area to do quiet activities to ensure all children receive the time they require for a nap.

I do not allow toys or other personal items, including costume jewelry, stuffed animals, etc., brought from home into the childcare for it creates conflict. If a personal item arrives with you and your child, you will be asked to take it with you.

As parents and providers, we must work TOGETHER to ensure that your child can develop to his/her potential in a warm, loving, and nurturing environment. It is important that we communicate effectively about your child. I will make it a point to keep you informed about your child’s physical, emotional, and social growth, as well as give you any possible and negative feedback related to these areas. Your input on any issue concerning your child is very much appreciated.

When you feel your child is ready for potty training, I ask that you begin teaching at home during a weekend or vacation. PLEASE NOTE: I will only assist your child in potty training if you have successfully begun training at home for one week prior and when a mutual agreement of when to start has been determined.

I will follow through and encourage your child while in care. Potty training will be done in a relaxed manner with the cooperation of the family. I require that the child be at least 2 years of age and must also show signs of readiness. Positive reinforcements and consistency must be continued at home.

The child must be kept in pull-ups at all times. Please keep in mind that the activity level at a childcare can distract your child from responding to an urge to use the potty, more so than at your home.

The following items are to be left at the childcare and replaced as needed. Soiled clothes will be washed at the end of the day and saved used to maintain a supply of clothing.

Two (2) changes of clothing, including socks (an extra pair of shoes if available). A pack of pull-ups—you will be notified when the supply is running low.

State regulations require immunization records be on file prior to care and kept up to date- Infants: every 6 months/ Toddlers: Annually / Preschooler: every 18 months/ School Age: every 3 years. My program requires families to be immunized, apart from any covid vaccination. Covid and influenza vaccines are not required in my setting.

If your child is allergic to food items a plan prior to starting will need to be put in place. If your child has seasonal environmental allergies bring these to my attention so a plan be in place. If your child is allergic to Bee’s, an epi pen will need to stay here and a plan in place for care along with a written doctor’s note will be needed.

If at any time your child is too sick or contagious based on the guidelines to participate in activities, I will notify you immediately. Parents or previously authorized persons will be required to pick up the sick child within 1 hour of the notification.

If your child’s physician verifies that your child has a contagious condition, you will need to notify us as soon as possible, but within 24 hours. I will let the other parents know of the contagious condition so they can be alert for similar symptoms.

If you feel that your child is not well enough to participate in all activities, including outdoor play, I require your child to stay at home.

The Health Department regulations prohibit the admittance of any child into a family childcare home that exhibits any of the following symptoms:

Fever (100 f. or higher) – child needs to be fever free, regardless of the cause, for 24 hours without using fever reducing medications.

Diarrhea – child must be symptom free for 24 hours

Vomiting – child must be symptom free for 24 hours

Runny nose with colored discharge – check with doctor

Rash – check with doctor

Discharge from eyes or ears

Lice – child needs to be treated before return (NO NITS AT ALL)

Communicable diseases – chicken pox, measles, mumps, conjunctivitis (pink eye), influenza, etc. The child may return when the incubation and contagious period is passed, and the child is well enough to resume normal activities

I have the right to refuse to care for a sick child.
The provider shall require that a child’s parent notify the provider within 24 hours of the diagnosis of a serious, contagious illness, or parasitic infestation listed below:

o Amebic Dysentery Pertussis (whopping cough)
o Brucellosis Psittacosis
o Encephalitis (all types)
o Reye’s Syndrome
o Food borne Illness
o Rocky Mountain Spotted Fever
o Hepatitis, Viral (A, B, Non-A, Non-B)
o Rubella
o Lead Poisoning
o Salmonellosis
o Leprosy
o Shigellosis
o Leptospirosis
o Trichinosis
o Malaria
o Tuberculosis
o Meningitis (all infectious types)
o Tularemia
o Meningococcemia
o Typhus
o Mumps
o Venereal Disease
o Occupationally Related Diseases
o Yellow Fever
o Ophthalmia Neonatorum
o Anthrax
o Cholera
o Plague
o Rabies
o Tetanus
o Botulism Measles (Rubella)
o Poliomyelitis Smallpox

The provider shall inform a parent of each exposed child the same day the provider is notified a positive diagnosis has been made for any of the illnesses or parasitic infections listed above.

The provider shall notify the health officer or Minnesota Department of Health of any suspected case of reportable disease as specified above.

I will give prescribed medication when a doctor indicates that they are no longer contagious. A signed release form from the physician and a written request and complete form filled out form you prior to care of the medication to be given is needed. The medication must be in the original container, labeled with proper dates, directions of dosage and times to be given and child’s name for.

Over the counter medications, creams and ointments can be given with the permission form completed prior for these such items: diapering products, sunscreen solutions, insect repellents, etc. Medications will be stored in a safe place inaccessible from children in my home. I ask that medications not be in a child’s personal travel bag that comes and goes from home. Fever reducing medication will not be given. ONLY pain relief medications will be given for relief of discomfort for shots, ear infections teething etc. All medications will go home after the prescribed date.

If a current family is seeking an additional slot due to a pregnancy or summer school age care they will have priority over part time families. A holding fee of $100 will secure this slot when you get closer to lock them in. Slots are filled by how best they fit my program and other program factors, and not by number order.

Parental Decision- A parent may decide to withdraw the child for any reason. I require a 2-week WRITTEN notice before discontinuation of services. Fees for the last 2 weeks of care are due upon starting.

Delinquent Account- Fees are due Friday, in advance of the week I am providing care. An account is considered delinquent when the payment for the week is not paid by Friday or by the last day of attendance.

Inability to meet the needs of the child- I will strive to provide for the needs of all children, however, in some instances, a child’s needs may not be able to be met by my childcare setting.
Abusive behavior by a child- Children who are persistently physically or verbally abusive to other children, us, or our property will be terminated.
